Letter of Counseling

Fitness Assessment Failure


1. Investigation has disclosed that on 22 September 2026, you participated in the Air Force Physical Fitness Readiness Assessment (PFRA) at the Fitness Assessment Cell on Joint Base Pearl Harbor-Hickam. You received an overall score of 32.5 points. IAW AFMAN 36-2905, Air Force Physical Fitness Program, this score is unsatisfactory (< 74.9) and resulted in a Not Ready determination. Additionally, you did not meet the minimum requirement of 42 shuttles for the 20-Meter High Aerobic Multi-shuttle Run (HAMR). You completed 29 repetitions, resulting in a score of zero (0) points for that component.

2. You are hereby counseled. As a Non-Commissioned Officer in the United States Air Force, maintaining physical readiness is essential to your ability to perform assigned duties, meet deployment and contingency requirements, and uphold the standards. Your failure to meet the established fitness standard is unacceptable. Physical fitness is a personal responsibility and directly affects unit readiness, mission capability, and your credibility as a Staff Sergeant. Your conduct is unacceptable and further deviation may result in more severe action. You are expected to take immediate and sustained action to correct this deficiency.

3. Effective immediately, you will:

Develop and provide a written fitness improvement plan no later than 05 October 2026.
Complete 3 additional fitness-training sessions per week, focused on the areas in which you did not meet the standard.
Provide progress updates weekly beginning 05 October 2026.
Participate in remedial fitness program as directed.
Meet with [UFPM/facility fitness staff/medical provider, as appropriate] by [date] to identify available resources and ensure there are no unresolved medical concerns affecting performance.
Successfully meet Air Force fitness standards during your next official assessment in December 2026.

4. The following information required by the Privacy Act is provided for your information. AUTHORITY: 10 USC § 9013. PURPOSE: To obtain any comments or documents you desire to submit (on a voluntary basis) for consideration concerning this action. ROUTINE USES: Provides you an opportunity to submit comments or documents for consideration. If provided, the written response you submit becomes a part of the record. DISCLOSURE: Your written acknowledgment of receipt and signature are mandatory. Any other comments or documents you provide are voluntary.

5. You will acknowledge receipt of this letter of counseling immediately by signing the first indorsement. Within 3 duty days from the date you received this letter, you will provide your response by signing the second indorsement below. Any comments or documents you wish to be considered concerning this letter must be submitted at that time. After receiving your response, I intend to notify you of my final disposition of this action within 3 duty days.
