Response to Letter of Counseling
Failure to Follow Directions

I, Aomyia Hireran, acknowledge this Letter of Counseling documentation. I did not uphold the standards of a certified ISRT member, nor did I follow the proper protocols that could have prevented and resolved this incident. I understand that if I am not sure of the required procedures during future shift changes, that I should ask questions and follow the established and clearly documented procedures. I will continue to use my chain of command and SOP to perform my duties of being a certified ISRT member properly.
